Introduction: 

At DAS Academy, the safety and well-being of our participants are of utmost importance. However, soccer is a physical sport, and as such, there are inherent risks involved. This waiver is intended to outline these risks and to explain the limitations of DAS Academy's liability. By signing this waiver, you acknowledge and agree to these terms and conditions. 

 

Acknowledgment of Risk: 

I, the undersigned parent/guardian, understand that participation in the DAS Academy Soccer Program involves risks including, but not limited to: 
 

1. Physical Injuries: These may include, but are not limited to, sprains, strains, fractures, concussions, and other injuries that may result from physical contact, falls, or the nature of the sport itself. 
 

2.Environmental Risks: Training conditions such as rain, heat, or cold may pose additional risks during training. 
 

3. Equipment Risks: Injuries may occur due to the use of soccer equipment, including but not limited to, balls, goals, and training apparatus. 
 

4.Travel Risks: Risks associated with traveling to and from training sessions, or other events organized by DAS 

Academy. 
 

Waiver of Liability: 

In consideration of my child's participation in the DAS Academy Soccer Program, I, the undersigned, agree to the following: 

1. Assumption of Risk: I acknowledge that my child’s participation in the soccer program is entirely voluntary and 

that I fully understand and accept the risks involved. 
 

2. Release of Liability: I, on behalf of myself, my child, and our heirs, assigns, and personal representatives, hereby release, discharge, and hold harmless DAS Academy, its officers, directors, employees, coaches, volunteers, agents, and affiliates (collectively referred to as “the Released Parties”) from any and all claims, demands, damages, expenses, or lawsuits that may arise out of or in connection with any injury, illness, or other damage that may occur to my child during their participation in the soccer program, whether arising from the negligence of the Released Parties or otherwise. 

3. Indemnification: I agree to indemnify and hold harmless the Released Parties from any and all liabilities, claims, demands, costs, or expenses, including reasonable attorney's fees, which may result from my child’s participation in the soccer program. 

4. Medical Treatment: In the event of an emergency, I authorize DAS Academy and its representatives to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for my child’s 

immediate care, and I agree to be responsible for payment of any and all medical services rendered.

 

5.  No Warranty: I understand that DAS Academy does not warrant or guarantee the safety of the facilities, equipment, or services provided and that participation is at my child’s own risk. 

Agreement to Rules and Conduct: 

I agree that my child will adhere to all rules, regulations, and instructions provided by DAS Academy staff. Failure to comply with these rules may result in my child’s removal from the program without refund or recourse
